ABSTRACT This study analyzed the timed up and go test (TUG) as a tool to evaluate muscle strength in community-dwelling older people. As a methodology, an observational, cross-sectional and analytical study was conducted. Data from 442 older people were analyzed, both women (71.7%) and men (28.3%). Ages ranged from 62- 104 years, with a mean of 80.85 (±8.047) years. TUG was used as a tool to evaluate muscle strength. Comparison and correlation analyses were performed to compare performance between the TUG and the chair stand test (CST) and between TUG and handgrip strength (HGS). Analysis of agreement between the diagnosis of low physical performance on TUG and the diagnosis of muscle weakness on CST and HGS was also performed. The predictive values were estimated using the receiver operating characteristic curve. The analyses showed a moderate direct proportional correlation between TUG and CST performance and low inversely proportional correlations between TUG and HGS performance and between CST and HGS (p<0.001). In the accuracy analyses, it was observed that TUG test could predict muscle strength in CST (S 34.4%; PPV 91.5%; NPV 26.1%; AUC=0.779; 95%CI 0.724-0.835; p<0.001) and handgrip dynamometry (S 44.2%; PPV 77.4%; NPV 67.2%; AUC=0.652; 95%CI 0.599-0.705; p<0.001). The Timed Up and Go test was able to satisfactorily indicate individuals with normal muscle strength in CST.

RESUMEN Este estudio analizó el uso de la prueba de levantarse y andar (TUG) como una herramienta para evaluar la fuerza muscular en adultos mayores residentes en la comunidad. Como metodología, se trata de un estudio observacional, transversal y analítico. Se analizaron datos de 442 ancianos, del sexo femeninol(71,7%) y masculino (28,3%), de edad de entre 62 y 104 años, y edad media de 80,85 (±8,047) años. El instrumento TUG se utilizó para evaluar la fuerza muscular. Se realizaron análisis de comparación y correlación entre el rendimiento en TUG y en la prueba de sentarse y levantarse (SL), y entre TUG y la fuerza de agarre. También se realizaron análisis de concordancia entre el diagnóstico de bajo rendimiento físico en TUG y el diagnóstico de debilidad muscular en SL y fuerza de agarre. Los valores predictivos se trazaron utilizando la curva receiver operating characteristic (ROC). Hubo una correlación directamente proporcional moderada entre el rendimiento de TUG y de SL, y se encontraron correlaciones inversamente proporcionales bajas entre el rendimiento de TUG y la fuerza de agarre, así como entre SL y la fuerza de agarre (p<0,001). En los análisis de precisión se observó que TUG fue capaz de predecir la fuerza muscular en SL (S 34,4%; VPP 91,5%; VPN 26,1%; AUC=0,779; IC 95% 0,724-0,835; p<0,001) y en la dinamometría de agarre (S 44,2%; VPP 77,4%; VPN 67,2%; AUC=0,652; IC 95% 0,599-0,705; p<0,001). Se concluye que TUG fue capaz de indicar satisfactoriamente a los individuos con fuerza muscular normal en SL.

RESUMO Este estudo analisou o uso do timed up and go test (TUG) como ferramenta de avaliação da força muscular em idosos da comunidade. Como metodologia, foi realizado um estudo observacional, transversal e analítico. Foram analisados dados de 442 idosos, do sexo feminino (71,7%) e do masculino (28,3%), com idades entre 62 e 104 anos, e média de 80,85 (±8,047) anos. O TUG foi utilizado como instrumento de avaliação da força muscular. Foram realizadas análises de comparação e correlação entre o desempenho no TUG e no teste de sentar e levantar (TSL), e entre o TUG e a força de preensão manual. Também foram realizadas análises de concordância entre diagnóstico de baixo desempenho físico no TUG e diagnóstico de fraqueza muscular no TSL e na força de preensão manual. Os valores preditivos foram traçados por meio da curva receiver operating characteristic (ROC). As análises mostraram correlação diretamente proporcional moderada entre o desempenho no TUG e no TSL e correlações inversamente proporcionais baixas entre o desempenho no TUG e a força de preensão manual e entre o TSL e a força de preensão manual (p<0,001). Nas análises de acurácia, observou-se que o TUG apresentou capacidade de prever a força muscular no TSL (S 34,4%; VPP 91,5%; VPN 26,1%; AUC=0,779; 95% IC 0,724-0,835; p<0,001) e na dinamometria de preensão palmar (S 44,2%; VPP 77,4%; VPN 67,2%; AUC=0,652; 95% IC 0,599-0,705; p<0,001). Conclui-se que o TUG apresentou capacidade de indicar de forma satisfatória os indivíduos com força muscular normal no TSL.

Abstract During a survey of mucoralean fungi in soil from an upland forest area located in Pernambuco, Brazil, a strain of Backusella (URM 8637) was isolated. Based on morphological, physiological, and molecular data [internal transcribed spacer (ITS) and large subunit (LSU) ribosomal DNA regions], it was recognized that this Backusella differed from all other species in the genus. Morphologically, the new species is characterized as forming varied-shaped columellae, including elongated, basally constricted, unisporate (rare) and multisporate sporangiola, and ellipsoidal sporangiospores. The maximum temperature growth of URM 8637 on malt extract agar and potato dextrose agar was 36 °C. In the phylogram, it was closely related to B. constricta. Based on the evidence from the analyzed datasets, a new species of Backusella is proposed. An updated identification key for Backusella from the Americas is provided.

ABSTRACT The analysis of transport demand is one of the main forms of public transport management and significantly impacts tax revenues and public management. This study evaluates the demand for intercity public transport in the Brazilian state of Rio Grande do Norte, from January 2019 to July 2022, in three categories of users: full tickets, half tickets, and gratuities. The hypothesis is that the integration of demand systems can identify underdeclaration bias by crossing declared demand data in the Transport Information Management System (GISIT) and the actual demand data from the Electronic Ticketing System obtained by the TDMax Reports software. Access to public transport databases was provided by the Departamento de Estradas de Rodagem do Rio Grande do Norte (DER/RN). The results reveal an underdeclaration of the demand for public transport in the state, observed in 41.74% of the fleet.

RESUMEN El análisis de la demanda de transporte es una de las principales formas de gestión del transporte público y provoca importantes impactos recaudatorios para la gestión pública. Así, este estudio tuvo como objetivo evaluar la demanda de transporte público interurbano en Rio Grande do Norte, de enero de 2019 a julio de 2022, en tres categorías de usuarios: boletos completos, medios boletos y gratuidades, a partir de la hipótesis de que la integración de los sistemas de demanda puede identificar el sesgo de declaración insuficiente al cruzar los datos de demanda declarados, presentes en el Sistema de gestión de información de transporte (GISIT), y los datos de demanda real del Sistema de emisión de boletos electrónicos, obtenidos por el software TDMax Reports. El acceso a las bases de datos de transporte público fue proporcionado por el Departamento de Carreteras de Rio Grande do Norte (DER/RN). Los resultados mostraron subdeclaración de la demanda de transporte público en RN, observada para el 41,74% de la flota.

RESUMO A análise da demanda de transportes é uma das principais formas de gerenciamento do transporte público e causa impactos arrecadatórios importantes para a gestão pública. Desse modo, este estudo objetivou avaliar a demanda do transporte público intermunicipal do Rio Grande do Norte, no período de janeiro de 2019 a julho de 2022, em três categorias de usuários: passagens inteiras, meias passagens e gratuidades, baseando-se na hipótese de que a integração dos sistemas de demanda pode identificar viés de subdeclaração ao cruzar dados declarados de demanda, presentes no Sistema de Gestão de Informações de Transportes (GISIT), e os dados reais de demanda do Sistema de Bilhetagem Eletrônica, obtidos pelo software TDMax Reports. O acesso aos bancos de dados do transporte público foi disponibilizado pelo Departamento de Estradas de Rodagem do Rio Grande do Norte (DER/RN). Os resultados demonstraram subdeclaração da demanda do transporte público do RN, observada para 41,74% da frota.

ABSTRACT Eckersley and Tobin (2019) and Grin and Abrucio (2018) highlight the lack of attention given to municipal state capacities in academic national, and international literature, despite the increasing decentralization of public policies. Against this backdrop, this study offers a bibliometric review of the scientific production related to the subject in the period from 1975 to 2020. Data was collected from the Web of Science database, and analyzed using the CitNetExplorer and VOSviewer software. The analysis provided a map with citations, authors, countries, sources, keywords, and bibliographic coupling, which made it possible to understand the theme in different contexts and situations and point out directions for future research.

Por lo tanto, el objetivo del presente estudio fue realizar una revisión bibliométrica de la producción científica relacionada con el tema, en el período de 1975 a 2020. Para ello, se realizaron búsquedas en la base de datos Web of Science, y los datos se analizaron mediante los softwares CitNetExplorer y VOSviewer. El análisis proporcionó un mapa con citas, autores, países, fuentes, palabras clave y acoplamiento bibliográfico, lo que permitió comprender el tema en diferentes contextos y situaciones y señalar direcciones para futuras investigaciones.

RESUMO Objetivo: Estimar a prevalência de aleitamento materno exclusivo (AME), introdução de água, chás ou outros leites, bem como identificar os fatores associados à interrupção do AME aos 30 dias de vida. Métodos: Estudo transversal que utilizou questionários estruturados e pré-testados, aplicados a 310 mães em dois momentos: presencialmente, na maternidade, e aos 30 dias de vida da criança, mediante ligação telefônica. Estatística descritiva e regressão multivariada de Poisson, seguindo modelo hierárquico multiníveis conforme a proximidade com o desfecho, estimaram a associação entre as variáveis dependentes e independentes. Resultados: A manutenção do AME aos 30 dias de idade da criança ocorreu em 85,2% da amostra, e 1,6 % receberam água, 11,5 % chás e 8,2% outro leite. Preditores para a interrupção do AME na análise univariada foram o retorno das mães ao trabalho ou estudo logo após o nascimento do bebê (razão de incidência — RI 2,88; intervalo de confiança — IC95% 1,14–7,25) e o uso de chupeta (RI 3,29; IC95% 1,52–6,22). A interrupção do AME foi menor no grupo de participantes que recebeu apoio da avó materna do lactente (RI 2,71; IC95% 1,11–6,56) e do companheiro (RI 4,78; IC95% 1,90–12,06). Após a análise multivariada, apenas o uso de chupeta (RI 5,47; IC95% 2,38–19,3) e o apoio do companheiro (RI 6,87; IC95% 2,04–23,1) mantiveram associação com o desfecho. Conclusões: A prevalência de AME encontrada neste estudo pode ser considerada boa, e futuras intervenções que visem ao aumento da duração do AME nessa população devem levar em consideração a participação do companheiro e o reforço para a não introdução da chupeta.

ABSTRACT Objective: To estimate the prevalence of exclusive breastfeeding (EBF), introduction of water, herbal teas, or other milks, as well as to identify the factors associated with the interruption of EBF at the 30th day after birth. Methods: This is a cross-sectional study using structured and pretested questionnaires applied to 310 mothers in two moments: in person, at the maternity ward; and at the time the infant was 30 days of age, by telephone call. Descriptive statistics and multivariate Poisson regression, following a multilevel hierarchical model according to the proximity to the outcome, were used to estimate the association between dependent and independent variables. Results: The maintenance of EBF at 30 days of age of the infant occurred in 85.2% of the sample, 1.6% receiving water, 11.5% herbal teas, and 8.2% other milk. Predictors for EBF interruption in the univariate analysis were the mothers’ return to work or study activities shortly after the baby's birth (IR 2.88; 95%CI 1.14–7.25) and the use of a pacifier (IR 3.29; 95%CI 1.52–6.22). The interruption of EBF was lower in the group of participants who received support from the infant's maternal grandmother (IR 2.71; 95%CI 1.11–6.56) and their partner (IR 4.78; 95%CI 1.90–12.06). After a multivariate analysis, only the use of a pacifier (IR 5.47; 95%CI 2.38–19.3) and the partner's support (IR 6.87; 95%CI 2.04–23.1) maintained the association with the outcome. Conclusions: The prevalence of EBF found in this study can be considered good, and future interventions aimed at increasing the duration of EBF in this population should take into account the participation of the partner and the reinforcement for not introducing the pacifier.

RESUMO: O objetivo do presente trabalho foi verificar o efeito in vitro da triiodotironina (T3) na diferenciação condrogênica de células tronco mesenquimais do tecido adiposo (CTM-TA) de ratas, durante vários períodos e em várias doses. CTM-TA foram coletadas de ratas Wistar e cultivadas em meio condrogênico com ou sem a presença de T3. Constitui-se cinco grupos: 1) CTM-TA sem T3; e 2,3,4,5) CTM-TA com T3 (0,01; 1; 100 e 1000 nM, respectivamente). Após sete, 14 e 21 dias, foram avaliados morfologia celular, formação de matriz condrogênica e expressão de Sox9, agrecano, colágeno II e colágeno X. Para as análises foi utilizado o teste de Student Newman Keuls. CTM-TA expressaram CD54, CD73 e CD90 antes da diferenciação condrogênica. O tratamento hormonal não alterou a formação de matriz condrogênica e a expressão de Sox9 aos 14 e 21 dias e expressão dos colágenos II e X em nenhum dos períodos avaliados. No entanto, as doses de 0,01; 1 e 1000 nM T3 diminuíram a expressão de Sox9 aos 7 dias. Conclui-se que a diferenciação condrogênica de CTM-TA de ratas não é influenciada pela T3.

ABSTRACT: The objective of this study was to investigate the in vitro action of triiodothyronine (T3) on the chondrogenic differentiation of adipose tissue-derived stem cells (ASCs) of female rats, with different time periods and doses. ASCs were extracted from female Wistar rats and were cultured in chondrogenic medium with and without the presence of T3. Five groups were established: 1) ASCs without T3; and 2,3,4,5) ASCs with 0.01, 1, 100 and 1,000 nM T3, respectively). After 7, 14 and 21 days, cell morphology, chondrogenic matrix formation, and expression of Sox9, aggrecan, collagen II, and collagen X were evaluated. The Student-Newman-Keuls test was used. ASCs showed CD54, CD73, and CD90 before chondrogenic differentiation. The hormone treatment did not alter chondrogenic matrix formation, Sox9 expression at 14 or 21 days, or expression of collagen II or collagen X at any time. However, the 0.01, 1, and 1000 nM T3 doses decreased Sox9 expression at 7 days. In conclusion, chondrogenic differentiation of ASCs of female rats is not influenced by T3.

RESUMO Este estudo teve como objetivo caracterizar a cadeia produtiva do leite e estudar a sanidade do rebanho leiteiro do município de Codó, estado do Maranhão, Brasil, através da aplicação de um checklist e da realização de exames para detecção de mastite clínica e subclínica pelos métodos da caneca do fundo escuro e California mastitis test (CMT), no período de junho a agosto de 2019, em 295 vacas leiteiras procedentes de 20 propriedades. Coletaram-se amostras de leite e água da ordenha para análises microbiológicas. Observou-se que os ordenhadores não possuem conhecimento adequado sobre as boas práticas agropecuárias. Quanto à realização da ordenha, apenas 60% realizam-na em currais com cobertura; quanto ao piso, 60% são de terra batida e 40% de concreto. A água utilizada no manejo da ordenha é proveniente de poços em 35% das propriedades e as demais de açudes. As práticas de pré e pós-dipping e os testes CMT e caneca de fundo escuro e a adoção de programa de controle da mastite não são realizados em nenhuma das propriedades. Diagnosticaram-se duas vacas com mastite subclínica e uma com tuberculose e verificou-se elevada contaminação por coliformes totais e termotolerantes nas análises microbiológicas do leite, variando entre 23 NMP/mL a >1.100 NMP/mL e < 3.0 a > 1.100 NMP/mL, respectivamente, e presença de estafilococos coagulase positivos em 25% das amostras. As amostras de água também apresentaram elevada contaminação por coliformes totais entre 4,1 NMP/mL a > 2.419,6 NMP/mL e 40% apresentaram presença de Escherichia coli. Esses resultados refletem a necessidade de mais investimentos em assistência técnica e treinamento técnico para esses produtores.

ABSTRACT In order to characterize the milk production chain and study the dairy herd health in the city of Codó, state of Maranhão, Brazil, a checklist was applied and tests were carried out to detect clinical and subclinical mastitis using mastitis test strip cup and the California mastitis test (CMT), from June to August 2019, in 295 dairy cows from 20 farms. Water and milk samples were collected for microbiological analysis. It was observed that herdsmen do not have adequate knowledge about good agricultural practices. As for milking, only 60% are performed in corrals with coverage, and as for the floor, 60% are made of clay and 40% are made of concrete. In 35% of the properties, the water used in milking management comes from wells and the rest from dams. Pre and postdipping practices, CMT, mastitis strip cup test and the adoption of a mastitis control program are not carried out on any of the properties Two cows tested positive for subclinical mastitis and one cow tested positive for tuberculosis. In the microbiological analysis of the milk, a high count of total coliforms and thermotolerants was obtained, with values between 23 to > 1,100 MPN/mL and < 3.0 to > 1,100 MPN/mL, respectively. The presence of coagulase positive staphylococci was also observed in 25% of the samples. The water samples also showed high contamination by total coliforms between 4.1 to > 2.419.6 MPN/mL and 40% showed the presence of Escherichia coli. These results reflect the need for more investments in technical assistance and technical training for these producers.

ABSTRACT The aim of this study was to evaluate the concordance between two versions of the scoring system (2011 and 2019), recommended by the Brazilian Ministry of Health, for the diagnosis of pulmonary tuberculosis (PTB) in children and adolescents. A retrospective descriptive study was performed to assess the medical records of children and adolescents with PTB, in TB units from Brazilian cities located in Rio de Janeiro, Minas Gerais, and Parana States, from January 1 st , 2004, to December 1 st , 2018. Patients aged 0 to 18 years old with a diagnosis of PTB were included. The comparison between the two scoring systems showed a moderate concordance according to the κ coefficient value = 0.625. Fourteen patients showed a reduction in the TB score, going from 30 points in the 2011, to 25 points or less in the 2019 one. Seventy one percent of these 14 patients had radiological changes suggestive of PTB and 86% had tuberculin skin tests greater than 10 mm. The study concluded that a moderate agreement was observed between the 2011 and 2019 scoring systems, with an increase in the number of patients scoring 25 points or less in 2019, which can eventually hinder the diagnosis of PTB.

Resumo Novas formulações de agregado de trióxido mineral (MTA) são constantemente introduzidas no mercado, geralmente em forma de pó e líquido. O Biocerâmico (Bio-C) Reparador (Repair) é um material pronto para uso sugerido como substituto do MTA, mas suas propriedades precisam ser estudadas. Este estudo avaliou a citotoxicidade, biocompatibilidade e biomineralização do Bio-C Repair comparado ao MTA-High Plasticity (MTA-HP) e MTA branco da Angelus (MTA-Ang). Fibroblastos L929 foram expostos a extratos dos materiais (não diluído, ½ e ¼ diluições; 6, 24 e 48 h). Tubos de polietileno contendo os materiais ou vazios (controle) foram implantados no tecido subcutâneo de ratos. Após 7 e 30 dias (n=8), os espécimes foram removidos para análises (hematoxilina-eosina, von Kossa e luz polarizada). Os dados da citotoxicidade foram analisados estatisticamente pelo teste de two-way ANOVA, e os dados da biocompatibilidade pelos testes de Kruskal-Wallis e Dunn (p<0,05). As células expostas aos materiais apresentaram maior viabilidade celular na maior parte dos períodos, comparados com o controle (p<0,05). O extrato não diluído e ½ diluição do MTA-HP apresentaram maior citocompatibilidade do que Bio-C Repair às 6h, e com ¼ diluição às 24h (p<0,05); o MTA-Ang branco apresentou maior citocompatibilidade do que o Bio-C Repair na maior parte dos períodos (p<0,05). O extrato não diluído do MTA-Ang branco apresentou maior citocompatibilidade às 6 e 24 h comparado ao MTA-HP, e com ½ diluição às 24h (p<0,05). A citocompatibilidade dos materiais foi semelhante às 48 h para a maior parte das diluições (p>0,05). Aos 7 e 30 dias, os grupos apresentaram inflamação moderada e leve, respectivamente (p>0,05). Todos os materiais mostraram estruturas positivas para von Kossa e luz polarizada. Em conclusão, o Bio-C Repair teve citocompatibilidade semelhante aos materiais à base de MTA, é biocompatível e induz à biomineralização.

Abstract New mineral trioxide aggregate (MTA) formulations are constantly introduced in the market, usually in a powder-and-liquid form. Bioceramic (Bio-C) Repair is a ready-for-use material suggested as substitute for MTA, but its properties need to be studied. This study evaluated the cytotoxicity, biocompatibility and biomineralization of Bio-C Repair compared to MTA Repair High-Plasticity (MTA-HP) and white MTA-Angelus (MTA-Ang). L929 fibroblasts were exposed to material-extracted (undiluted, ½ and ¼ dilutions; 6, 24 and 48h). Polyethylene tubes with material or empty (control) were implanted in the subcutaneous tissue of rats. After 7 and 30 days (n=8), the specimens were removed for analysis (hematoxylin-eosin, von Kossa and polarized light). Cytotoxicity data were statistically analyzed by two-way ANOVA, and biocompatibility data by Kruskal-Wallis and Dunn tests (p<0.05). The cells exposed to the materials had greater viability at most of the periods compared with control (p<0.05). The undiluted and ½ dilutions of MTA-HP extract showed higher cytocompatibility than Bio-C Repair at 6 h and with the ¼ dilution at 24 h (p<0.05); the white MTA-Ang showed higher cytocompatibility than Bio-C Repair at most of periods (p<0.05). The undiluted white MTA-Ang extract had higher cytocompatibility at 6 and 24h than MTA-HP, and with ½ dilution at 24h (p<0.05). The materials’ cytocompatibility was similar at 48h for most dilutions (p>0.05). At 7 and 30 days, the groups had moderate and mild inflammation, respectively (p>0.05). All materials showed positive structures for von Kossa and polarized light. In conclusion, Bio-C Repair had similar cytocompatibility to MTA-based materials is biocompatible and induces biomineralization.

Abstract Objective: The aim of the present study was to evaluate the ability of ischemic postconditioning, atorvastatin and both associated to prevent or minimize reperfusion injury in the lung of rats subjected to ischemia and reperfusion by abdominal aortic clamping. Methods: We used 41 Wistar norvegic rats, which were distributed into 5 groups: ischemia and reperfusion (I/R), ischemic postcondictioning (IPC), postconditioning + atorvastatin (IPC+A), atorvastatin (A) and SHAM. It was performed a medium laparotomy, dissection and isolation of the infra-renal abdominal aorta; except for the SHAM group, all the others were submitted to the aortic clamping for 70 minutes (ischemia) and posterior clamp removal (reperfusion, 70 minutes). In the IPC and IPC+A groups, postconditioning was performed between the ischemia and reperfusion phases by four cycles of reperfusion and ischemia lasting 30 seconds each. In the IPC+A and A groups, preceding the surgical procedure, administration of 3.4 mg/day of atorvastatin was performed for seven days by gavage. After the surgical procedure, the right caudal lobe was removed from the lung for histological study, using tissue injury score ranging from grade 1 (normal tissue) to grade 4 (intense lesion). Results: The mean lung injury was 3.6 in the I/R group, 1.6 in the IPC group, 1.2 in the IPC+A group, 1.2 in the A group, and 1 in the SHAM group (P<0.01). Conclusion: Ischemic postconditioning and atorvastatin were able to minimize lung reperfusion injury, alone or in combination.

ABSTRACT: Extremely kaolinitic soils of Tertiary age elevations on the Brazilian east coast present a wide range of texture, which is recognized as one of the main factors controlling the soil organic matter contents. This study aimed to investigate the organic C storage of different compartments of kaolinitic soils. The studied soils had a wide particle size gradient, were under native forest vegetation, and located on Brazil's eastern coast (Coruripe - CF, Umbaúba - UF, Nova Viçosa - VF, Sooretama - SF, and Itaboraí - IF). The forest cover of all sites allows to record soil properties reference values for a land use condition closer to that of the original sites. We determined soil organic C (SOC) content and SOC stock up to a depth of 1 m, C of topsoil (0.00-0.08 m), aggregate size classes, and dissolved organic carbon (DOC) of the soil surface horizon (A horizon). Soil C stocks at the 0.00-1.00 m depth ranged from 105 to 137 Mg ha−1 and were not regulated by soil texture. The SF soils stored more C up to a depth of 1 m, while lower mean C stocks were found for UF and CF soils. Soil texture was not a reliable index to predict the C contents of the aggregate size classes of the 0.00-0.08 m layer (within each class and in total, using equivalent soil mass of the classes). The most clayey soils had a high percentage of 2-4 mm aggregates and, as a consequence, high aggregate stability indices, which are positively correlated to silt plus clay contents of the soil surface horizon. The proportion of DOC in relation to the total organic C of the surface soil horizon was high for IF and UF areas, which are the less preserved forest fragments among all studied fragments.

OBJECTIVES: Most thyroid diseases are nodular and have been investigated using ultrasound-guided fine needle aspiration biopsy (FNAB), the reports of which are standardized by the Bethesda System. Bethesda category III represents a heterogeneous group in terms of lesion characteristics and the malignancy rates reported in the literature. The objective of the present study was to evaluate the differences in the malignancy rates among Bethesda III subcategories. METHODS: Data from 1,479 patients who had thyroid surgery were reviewed. In total, 1,093 patients (89.6% female, mean age 52.7 (13-89) years) were included, and 386 patients were excluded. FNAB results (based on Bethesda Class) and histopathological results (benign or malignant) for coincident areas were collected. Bethesda III patients were subcategorized according to cytopathological characteristics (FLUS: follicular lesion of undetermined significance, Bethesda IIIA; AUS: atypia of undetermined significance, Bethesda IIIB). Data were correlated to obtain the malignancy rates for each Bethesda category and the newly defined subcategory. RESULTS: FNAB results for these patients were as follows: Bethesda I: 3.1%; Bethesda II: 18.6%; Bethesda III: 35.0%; Bethesda IV: 22.1%; Bethesda V: 4.1%; and Bethesda VI: 17.1%. The malignancy rates for Bethesda Class IIIB were significantly higher than those for Bethesda Class IIIA (p<0.001) and Bethesda Class IV (p<0.001). Bethesda Class IIIA showed significantly lower malignancy rates than Bethesda Class III overall (p<0.001) CONCLUSIONS: Improvements of the Bethesda System should consider this subcategorization to better reflect different malignancy rates, which may have a significant impact on the decision-making process.

OBJECTIVE: To evaluate the effectiveness and safety of first-generation protease inhibitors for the treatment of genotype 1 hepatitis C virus-infected patients at Brazilian reference centers. METHODS: This multicenter cross-sectional study included hepatitis C virus genotype 1 monoinfected patients treated with Peg-interferon, ribavirin, and either boceprevir (n=158) or telaprevir (n=557) between July 2013 and April 2014 at 15 reference centers in Brazil. Demographic, clinical, virological, and adverse events data were collected during treatment and follow-up. RESULTS: Of the 715 patients, 59% had cirrhosis and 67.1% were treatment-experienced. Based on intention-to-treat analysis, the overall sustained viral response was 56.6%, with similar effectiveness in both groups (51.9% for boceprevir and 58% for telaprevir, p=0.190). Serious adverse events occurred in 44.2% of patients, and six deaths (0.8%) were recorded. Cirrhotic patients had lower sustained viral response rates than non-cirrhotic patients (46.9% vs. 70.6%, p<0.001) and a higher incidence of serious adverse events (50.7% vs. 34.8%, p<0.001). Multivariate analysis revealed that sustained viral response was associated with the absence of cirrhosis, viral recurrence after previous treatment, pretreatment platelet count greater than 100,000/mm3, and achievement of a rapid viral response. Female gender, age>65 years, diagnosis of cirrhosis, and abnormal hemoglobin levels/platelet counts prior to treatment were associated with serious adverse events. CONCLUSION: Although serious adverse events rates were higher in this infected population, sustained viral response rates were similar to those reported for other patient cohorts.

Resumo Objetivo: descrever incidência de quedas em idosos no município de Barbacena, MG, com seus fatores causais, circunstâncias e consequências. Método: estudo de corte transversal, realizado mediante aplicação de questionários em 206 pacientes acima de 60 anos de idade, de novembro 2014 a fevereiro de 2015. Foram analisados fatores de risco, incidência e consequência das quedas na vida dos idosos. Foram construídas distribuições de frequência e calculadas as médias, desvios-padrão e percentagens. A existência de relação entre o relato de quedas e seus possíveis fatores de risco foi determinada por Teste qui-quadrado e exato de Fischer conforme a indicação. Resultados: observou-se a incidência de 36,41% de queda em idoso, sendo que 45,95% ocorreram fora de casa, 85,71% dos pesquisados já haviam sofrido acidente vascular encefálico (AVE) e 39,78% faziam uso de medicamento. Dos idosos que caíram e sofreram fratura (18,67%), 50% já tinham sofrido episódio de AVE e 50% eram portadores de doença renal crônica, sendo que 61,54% deixaram de realizar suas atividades diárias após a queda. Conclusão: a incidência de queda em idosos foi de 36,41%. Os fatores mais correlacionados foram uso de medicamentos, vítimas de AVE, portadores de doença renal crônica e, dos que sofreram fratura, 61,54% deixaram de realizar atividades diárias. Desta forma, a prevenção das quedas é uma preocupação de saúde pública e mudanças relativamente simples podem reduzi-las.

Abstract Objective: to determine the incidence of falls among the elderly population of the city of Barbacena in the state of Minas Gerais, together with causal factors, circumstances and major consequences. Methods: a cross-sectional study was performed through questionnaires applied to 206 patients over the age of 60, from November 2014 to February 2015 in the city of Barbacena, in the state of Minas Gerais. Risk factors related to falls were analyzed, as well as the incidence of falls and the consequences for the lives of elderly persons. The existence of a relationship between the reporting of falls and possible risk factors was determined by the Chi-squared and Fischer's exact tests as indicated. Results: an incidence of falls of 36.41% was observed among the elderly, 45.95% of which occurred outside the home. A total of 85.71% of respondents had previously suffered strokes and 39.78% were taking medication. Among elderly persons who have fallen and suffered fractures (18.67%), 50% had suffered strokes, 50% were suffering from chronic kidney disease, and 61.54% could not perform their activities of daily living after the fall. Conclusion: it was concluded that the incidence of falls among the elderly was 36.41%, while the most correlated factors were drug use, stroke victims and people with chronic kidney disease. Among those who suffered fractures, 61.54% failed to perform activities of daily living. Preventing falls is a public health concern, and simple changes can reduce its prevalence.

RESUMO OBJETIVO Caracterizar o processo de seleção de medicamentos na atenção primária à saúde, nas regiões brasileiras. MÉTODOS Artigo integrante da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços 2015, um estudo transversal que envolveu o levantamento de informações numa amostra de municípios das cinco regiões do Brasil. Os dados utilizados foram coletados por meio de entrevistas com os responsáveis pela assistência farmacêutica (n = 506), profissionais que realizam a dispensação de medicamentos (n = 1.139) e médicos (n = 1.558). Para avaliar a diferença entre as proporções foi adotado o teste do qui-quadrado para amostras complexas. As diferenças entre as médias foram analisadas em modelos lineares generalizados, com teste F com correção de Bonferroni para comparações múltiplas. Foram significativas as análises com valores de p ≤ 0,05. RESULTADOS Os responsáveis da assistência farmacêutica relataram não haver Comissão de Farmácia e Terapêutica formalmente constituída (12,5%). Eles afirmaram possuir uma Lista de Medicamentos Essenciais (85,3%) atualizada (80,4%), e serem participantes deste processo (88,2%). Contudo, na percepção dos entrevistados a Lista atende apenas parcialmente (70,1%) as demandas de saúde. Dos profissionais que realizam a dispensação de medicamentos entrevistados, apenas 16,6% eram farmacêuticos, mesmo assim, 47,8% referiram conhecer os procedimentos para mudança da lista. Na perspectiva da maior parte desses profissionais (70,9%), a lista atende as demandas de saúde do município. Dentre os médicos apenas 27,2% declararam ter conhecimento dos procedimentos para mudança da lista, mas 76,5% teriam alguma reivindicação de alteração. A maior parte reporta que baseia suas reivindicações em experiências clínicas (80,0%); para 13,0% deles, a lista atende às demandas de saúde. CONCLUSÕES Por tratar-se da primeira pesquisa nacional de caracterização do processo de seleção de medicamentos no âmbito da atenção primária à saúde, traz dados inéditos para a avaliação das políticas relacionadas com medicamentos no Brasil.

ABSTRACT OBJECTIVE To characterize the process of selection of medicines for primary health care in the Brazilian regions. METHODS This article is part of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços, 2015 (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines – Services, 2015), a cross-sectional study that consisted of an information gathering in a sample of cities in the five regions of Brazil. The data used were collected by interviews with those responsible for pharmaceutical services (PS) (n = 506), professionals responsible for the dispensing of medicines (n = 1,139), and physicians (n = 1,558). To evaluate the difference between ratios, we adopted the Chi-square test for complex samples. The differences between the averages were analyzed in generalized linear models with F-test with Bonferroni correction for multiple comparisons. The analyses considered significant had p≤0.05. RESULTS The professionals responsible for pharmaceutical services reported non-existence of a formally constituted Pharmacy and Therapeutics Committee (PTC) (12.5%). They claimed to have an updated (80.4%) list of Essential Medicines (85.3%) and being active participants of this process (88.2%). However, in the perception of respondents, the list only partially (70.1%) meets the health demands. Of the interviewed professionals responsible for the dispensing of medicines, only 16.6% were pharmacists; even so, 47.8% reported to know the procedures to change the list. From the perspective of most of these professionals (70.9%), the list meets the health demands of the city. Among physicians, only 27.2% reported to know the procedures to change the list, but 76.5% would have some claim to change it. Most of them reported to base their claims in clinical experiences (80.0%). For 13.0% of them, the list meets the health demands. CONCLUSIONS As this is the first national survey of characterization of the process of selection of medicines within primary health care, it brings unpublished data for the assessment of policies related to medicines in Brazil.
